Osteoporosis care is scattered by nature — a fracture seen in orthopedics, a DEXA scan filed in radiology, a drug regimen tracked nowhere but a paper chart. Fall risk, dietary history and quality of life rarely make it into any of them. That fragmentation makes it hard to see the whole patient, and nearly impossible to study treatment response over years.

Fracture Details
Fragility fracture history and sites, plus family history of fracture and osteoporosis.
Medical History
Celiac disease, lactose intolerance, diabetes, steroid intake and other relevant history.
Personal History
Exercise, smoking, alcohol use and maximum documented lifetime height.
Risk Factor for Fall
Falls in the last year, vision problems and balance issues.
Dietary History
Daily milk intake and milk-product consumption.
Treatment History
Calcium and vitamin D supplementation, and osteoporosis drug therapy — Zoledronate, Alendronate, Risedronate, Ibandronate, Denosumab, Teriparatide, Romosozumab — with route, dosing and duration.
Examination
Height, weight, BMI, kyphosis and lower-limb shortening.
Radiology
VFA and TBS (trabecular bone score), plus attached imaging.
Quality of Life
EQ-5D-5L — mobility, self-care, usual activities, pain-discomfort, anxiety/depression, and overall health score.
Expenditure
Consultation, medicine, investigation, physiotherapy, travel and lodging costs.
Investigation Follow-ups
Calcium, phosphate, albumin, creatinine, ALP, iPTH, 25(OH)D, PINP, CTX, height, weight and Hb — tracked visit over visit.
DEXA (Dual-energy X-ray Absorptiometry)
BMD, T-score and Z-score at L1-L4, femoral neck, total hip and 33% radius — tracked visit over visit.
